Procedures for changing contact information due to marriage, relocation, etc.

If your registration details change due to moving, transfer, marriage, etc., please provide us with your new contact information.

How to submit a notification

Notification by mail or fax

Please fill out the necessary information below and submit it to the Social Collaboration Promotion Division (Alumni Association Section).

<Required items>

• Name (if you have changed your name, please include your maiden name)
• Address
• telephone number
• date of birth
• Graduation year and department
